Cup Size vs. Implant Size
There is not a direct correlation between cup size and implant size.

The size of the implants is based on a combination of your goals and objectives and your anatomy. In order to provide a natural looking augmentation, Dr. Reddy needs to evaluate many factors including soft tissue coverage, skin laxity, chest wall width, breast imprint width, the shape of the breasts and relative level of constriction to ensure that the breast implants are appropriate for your body.
Saline Breast Implants Vs. Silicone Breast Implants
Generally, the first decision Dr. Reddy and the patient need to make together is whether to opt for Saline Breast Implants or Silicone Gel Breast Implants.
The advantage of silicone gel breast implants is primarily aesthetic: they look and feel so soft and natural that they typically cannot be distinguished from breasts without implants provided they do not develop a capsular contracture. They also have a lower rate of rippling and wrinkling. Also, because silicone is lighter than saline, the risk of downward displacement due to gravity is lower.
There are advantages of saline breast implants. They are less expensive (by about $1000- $1500) and frequently require a shorter scar.
Silcone gel implants frequently require a longer scar as they are pre-filled by the manufacturer, so they must be able to fit through the incision. In general, larger implants require longer scars. The length of the scar may be shortened with the use of a keller funnel, but this does add a nominal extra cost.
The risk of capsular contracture is slightly less with saline breast implants. However, this risk has been reduced since the breast implant companies are now including capsular contracture as part of their warranties for patients having primary breast augmentations.
This is because silicone implants might incur a late capsular contracture due to implant rupture whereas saline implants do not generally form capsular contractures in response to rupture or deflation. Finally, silicone gel implants might rupture “silently”, such that there is no outward evidence that a rupture has occurred. Physical exam by Dr. Reddy will identify about 30% of ruptures, whereas MRI will identify about 90% of ruptures, so women with silicone gel breast implants may consider MRI scans.
High Profile Breast Implants vs. Moderate Profile Breast Implants vs. Moderate Plus Profile Breast Implantsilicone gel implants have a natural upper pole slope when placed in the dual plane technique. High profile implants in general, in my opinion, give women a tapered, less matronly and a more thin appearance in the upper chest region. They are fuller and have more projection.
High profile silicone gels are useful in patients who have multiple capsulectomies, where visibility and rippling become a problem along the medial cleavage area. Although it may not completely remove the visibility and rippling, it may reduce it significantly versus textured implants as well as saline implants. The majority of the implants used in your practice are high profile saline and silicone implants because 1) again, they leave women with a more tapered appearance; 2) less visibility and rippling; and 3) more enhanced AP projection.
High profile implants come in both silicone gel and saline breast implants through Allergan and Mentor Corporation. The high profile implants have increased anterior/posterior projection and have a reduced lateral width or diameter. These implants, both saline and silicone, have reduced rippling and visibility due to a rounder shape to the breast implant.
The high profile silicone gel implants have a natural upper pole slope when placed in the dual plane technique. High profile implants are fuller and have more projection. Therefore, high profile breast implant will provide good cleavage and some lift (but in some cases a patient may still require a breast lift).
High profile silicone gels are useful in patients who have multiple capsulectomies, where visibility and rippling become a problem along the medial cleavage area. For the Natrelle Inspira breast implants, they use a designation SRF for their smooth round full breast implants. This is their equivalent to high profile.
Moderate plus profile implants are a compromise between the moderate profile (the widest and flattest implants, the least projection) and the high profile saline or silicone implants (the most projection and least in diameter or width). The moderate plus saline and silicone implants have a mid range of anterior/posterior projection and are not quite as wide as the more flattened moderate profile implant.Moderate plus profile implants are useful in patients who do not want too much upper pole fullness; however, do not want to have too much lateral width as well. These implants are excellent in the cohesive gels (also known as "gummy bears" breast implants) as well. The biggest advantage of moderate plus profile implants is not too much projection, especially useful for patients who are somewhat narrow along the thoracic cavity and those who do want a slightly less upper pole fullness that would found on the high profile saline and silicone gels.Moderate profile saline and silicone implants are the most commonly requested breast implants. They have less projection than high profile breast implants so they tend to look more natural on most women.
Breast Implant Companies
Dr. Kris Reddy FACS, West Palm Beach board certified plastic surgeon, utilizes the breast implants from the three companies that are approved by the FDA for use in the United States.
Natrelle (part of the Allergan company) manufacture both saline implants and silicone-filled implants. They are most commonly requested breast implants for breast augmentation which includes both primary breast augmentation to increase breast size, as well as revision surgery to correct or improve the result of a primary breast augmentation surgery.
Mentor is also another excellent source for silicone gel and saline breast implants. Mentor has sizing and profiles that are distinct from Natrelle so Dr. Reddy has the ability to better satisfy a patient's goals/epectations driven by personal preference, anatomy, and health.
Sientra specializes in cohesive gel implants (also known as gummy bear breast implants). There 5th generation implants are especially beneficial for patients who seek upper pole fullness. Note: the new Natrelle Inspira breast implants are also benefical for women seeking upper pole fullness.
Incision & Breast Implant Placement
When determining where to place the breast augmentation incision, Dr. Reddy considers your desires and the implant type being used. He typically places the incisions either around the areola or under the breast, because these locations result in minimal scarring for most patients. Silicone implants usually require larger incisions than saline implants.
Dr. Kris Reddy typically places the incisions either in the breast fold or the periareolar (around the areola).
Dr. Kris Reddy utilizes the incision in the breast fold for patients to maximize symmetry. This incision point allows for precise placement of the implant and minimal tissue disruption. If you need to exchange your implants in the future, your existing scar can be used.
For the periareolar placement, Dr. Reddy places a small incision around the nipple that heals well. The advantage of this placement is that the scar usually blends into the color transition of your areola. It is not a good option for patients with small areolas.
Some patients may have read about an axillary placement where an incision is made in the armpit endoscopically to create a pocket. Dr. Reddy will not perform this approach as it is riskier than the other approaches and may lead to significant medical issues. If revision surgery is necessary, you may need a secondary incision to remove or replace your implant.
The implants can be placed either between the breast tissue and the chest muscle (submammary also known as subglandular placement) or below breast tissue and the chest muscle (submuscular also known as subpectoral placement). The most common placement is subpectoral because this positioning allows for more complete coverage of the breast implants leading to generally more natural feel/look of the implants in the long-term. This position will also decrease the potential for rippling and/or palpability of the implants (which may increase with time, weight loss, and/or post-pregnancy changes).
The submuscular positioning also tends to interfere with mammography less so than in the sub glandular position. The incidence of breast implant encapsulation (capsular contraction) is also decreased with implants placed in the sub muscular position.
Round vs Teardrop Shape
Dr. Kris Reddy FACS utilizes round implants which provide the upper breast with the most fullness and shape. Round implants also will not rotate. Dr. Reddy will not utilize teardrop breast implants since they can move or rotate, distorting the shape of the breast and requiring the patient to have expensive revision surgery.
Keller Funnel in West Palm Beach

Cone shaped and made of nylon, the hydrophilic coating inside the funnel becomes extremely slippery when submerged in a saline or sterile solution.
Once the implant is placed inside the funnel, the surgeon places the small distal end into the prepared breast pocket and, using a pushing/squeezing motion, easily propels the implant into place.

Answer: Most of our patients like to come in 2 - 3 times prior to breast augmentation surgery. What are the most common local complications and adverse outcomes with breast implants?
The most common local complications and adverse outcomes associated with breast implants are capsular contracture, reoperation, implant removal, and rupture or deflation of the implant.
After your breast implant surgery, your breasts will begin to heal and adapt to the presence of the breast implants. A regular part of this process is that the breast tissue typically forms an internal scar immediately surrounding the implant. In many cases, this tissue forms a capsule that helps hold the implant in place. However, in some women, the scar tissue around the implant tightens and squeezes the implant. When scar tissue squeezes an implant, it is called capsular contracture. Capsular contracture causes the breast to feel abnormally firm and can cause pain. There is a scale for describing the severity of the contracture. It is called the Baker Grading Scale.
Capsular contracture may be more common if you have had a breast infection, hematoma (a solid swelling of clotted blood within the tissue) or seroma (collection of fluid that builds up under the surface of your skin). The chances of having contracture typically increase the longer you have your implants. Capsular contracture is a risk factor for implant rupture, and it is one of the most common reasons for reoperation. It also seems that women who have additional surgery to replace their implants (revision surgery) are more likely to have capsular contracture than women having their first augmentation or reconstruction.
It is likely that you will need additional surgery (a reoperation) at some point after your first breast implant surgery, either to correct a problem with or replace your breast implants. Patients may decide to change the size or type of their breast implants, requiring additional surgery. Problems such as rupture, capsular contracture, asymmetry (lack of proportion of shape, size and/or position between the two breasts), hypertrophic scarring (irregular, raised scar), infection, and shifting can require additional surgery. Some changes to your breast(s) after having breast implants are irreversible (cannot be changed or fixed). These may include dimpling, puckering, wrinkling, or the appearance that the breast is empty or deflated. Implant Removal Your breast implants may be removed (with or without being replaced) at some point during the course of your life. You and Grade I Grade II Grade III Grade IV Contracture is observed, but the breast feels and looks normal (it is soft) The breast is a little firm, but looks normal The breast is firm and looks abnormal The breast is hard, painful, and looks abnormal your doctor may decide to remove an implant or implants because of a complication or to improve the cosmetic result. Because these are not lifetime devices, the longer you have your breast implants, the more likely it will be for you to have them removed for any reason, either because of dissatisfaction, an unacceptable cosmetic result, or a complication such as severe capsular contracture.
Breast implants are considered to have ruptured when the implant shell develops a tear or hole. Implants could rupture any time after your implant surgery, but the longer the implants are in place, the higher the possibility that the implants will rupture or the gel will leak. Breast implants may rupture or leak because of any of these reasons:
• Damage by surgical instruments at the time of implantation or during any subsequent surgical procedure,
• Stress to the implant during implant surgery that weakens it,
• Folding or wrinkling of the implant shell,
• Excessive force to the chest (for example, during closed capsulotomy, which is a procedure that should not be used),
• Trauma (like being in a car accident),
• Compression during a mammogram,
• Severe capsular contracture,
• Normal use over time.
What is BIA-ALCL?
Breast Implant-Associated Anaplastic Large Cell Lymphoma (BIA- ALCL) is a rare type of lymphoma that has been found in women with breast implants.
It usually appears as a swelling of the breast caused by fluid surrounding the implant, usually occurring at least one year after surgery. Other late onset symptoms may include pain, lumps, swelling, or asymmetry. The recommended treatment is the removal of the breast implant. Treatment has been successful when caught early.
BIA-ALCL develops over several years (average 8 years, range 2 to 28 years). It is most commonly associated with textured breast implants which is one reason why Dr. Reddy does not utilize textured breast implants. This disease can also present as a lump in the breast or the lymph node in the armpit.
How often does BIA-ALCL occur?
Health authorities globally state the incidence of BIA-ALCL is rare.
In March of 2017, the U.S. Food and Drug Administration (FDA) reported they had received a total of 359 medical device reports (MDRs) of BIA-ALCL. The FDA had previously estimated that there were 5-10 million women with breast implants worldwide.
The FDA has noted that “BIA-ALCL occurs more frequently following implantation of breast implants with textured surfaces.”
Health authorities state that BIA-ALCL is very rare; it has occurred in only a very small number of the millions of women who have breast implants.
The FDA notes that symptoms of BIA-ALCL are typically late onset, meaning they occur at least one year after your surgery. Symptoms typically include pain, lumps, swelling, or asymmetry.
